How to refer a person seeking mental health services

We understand the importance of easily accessible and streamlined mental health services.


Our 'no wrong door' policy makes it easy for healthcare professionals and referrers to make a referral with confidence that their clients will receive timely support.

To make a referral, please follow the steps below. If you have any questions about the process, do not hesitate to contact our friendly team.

Prepare a referral letter

To ensure we can process referrals and provide timely care, please ensure you provide all of the information below as part of your referral submission.

Referring doctor's name, contact, provider number, and signature

Address the letter to “Dear Brain & Mind Hub” if you would like our specialist team to determine the most appropriate treatment pathway (very suitable for complex clients)

Specify the service/program being referred to (if known) e.g. Ketamine treatment, TMS

For Psychiatry referrals*, address to “Dr Psychiatrist” and indicate "Opinion & Report (291)" or "Opinion & Management (296)"

For Lifestyle Medicine referrals, address to “Dr Phoebe Slape”

Client’s medical history

Client's presenting concern and diagnosis (if known)

Past treatments for the condition

Current medications

Consent for us to contact your patient/client

Any relevant information to support the delivery of culturally safe and respectful care

Clinical triage team assessment

Our clinical triage team will assess each client's suitability, determine if further information is needed, and decide whether the referral can be accepted. If accepted, the client will be contacted for an intake call, and an initial comprehensive assessment appointment will be scheduled. If the referral cannot be fulfilled, we will notify the referring doctor directly to arrange alternative options.

Note: Referrals from a GP are valid for 12 months. Referral from a specialist are valid for 3 months. Discharge summaries from a hospital are valid for 3 months.
